I guess I was thinking of something along the lines of a door that says it leads to Prime World X or Y when "examined". It would not actually have to go to a server area. As others pointed out, if a PC takes such a portal, it effectively means they leave the server. It would provide some rp opportunities for folks to "return home" and then come back to Sigil.
I am surprised at the number of PCs whose backstory has to do with being trapped in Sigil, tricked into coming to Sigil, or who are otherwise "lost" in Sigil.
Sigil is not a prison (aside from Mazes), and folks could return to their home planes without going on a grand quest.
In any case, the World Serpent Inn connects many planes, not just Primes.

The problem there simply lies with the vast quantity of "Prime" worlds. If Toril, Oerth, and the other "canon" worlds held some great importance in the grand scheme of things, that might be true. But firstly, it's not like every portal in Sigil is perfectly mapped out, where a prime can walk up to a big board with a notation of every portal and a big "You Are Here" on it. Many of the portals are in places only accessible by members of specific factions, while others might link to a different location from one day to the next at the whim of the Lady. Not to mention that just because a portal might be found to Toril, it doesn't mean it would lead somewhere the person wanted to do. Not to mention about the portal keys, and how to use them.
For example one portal 'key' could require the following; first hopping on your left leg for three times, scratching your nose, and finally leaning against a nearby wall to drop a stone out of your boot - for the portal to open.

But its a moot point, don't have a Lady of Pain model, and its prolly best that way
(I am considering having her Herald in some future events though... she does on occation use mouth pieces to speak for her after all)
