Application for registration of a charity

Birthe would pay a visit to the City Court, seeking to either make an appointment in order to or, if such does not take too long, wait for an opportunity to file the required paperwork for establishing a charity.
For reference, she carries the information received from Garen Seph. If any forms are provided, she'd fill in the details to the best of her ability while waiting/prior to the appointment.
Name of Charity: Vana
Founder & sole administrator: Birthe
Purpose: Provision of food and shelter for the neediest citizens, to be expanded to further community services if funds permit
Location: Sigil, The Hive, <a street address>
Included with the paperwork is a copy of a deed for an old tavern not to deep in the Hive. If necessary, Birthe will swear she came to possess the deed after negotiations with a local and agreeing on terms that to her knowledge reflect common business practices in the area. (Unless there's objections, I'd go with Agony Aunt's suggestion the property be worth 5k gold.)
As initial funding is purely from private funds, there are no donations to declare at this time.
There will at this time be no employees, merely local volunteers.
Birthe requests help with preparing the paperwork to be filed with the Takers, specifically how to correctly ask for initial assessment of the property's value.
She carries the required gold for Fraternity paperwork with her. (And can hand that over to a DM if one would be so kind as to rob her blind take care of the charges.)
